I think Burt hurlers in Donegal won the county c'ship every year from 1991 and 2006 - 16 in a row. They must be up there.

Burt is the correct answer, 16 titles, however Mullahoran Hurling club in cavan could claim this feat, as they Officially been Cavan Champions every year from 1990 to 2007 (18 titles) but due to shortage in numbers in other clubs in the county, the Cavan Hurling Championship was not played for between 1995 and 2002, and during these years Mullahoran entered the Ulster Club Championship as Cavan Champions.

The late great Eamon Coleman is the man in question, played for Ballinderry one weekend, then Athlone the next, and in typical cheeky Coleman fashion got found out and banned!!! Not quite sure of the year, definately very early 80s.
